New Passports for Minors Under Age 16

STEP I.  GATHER APPLICATION DOCUMENTS

1.  DS-11 Application

Click here to create DS-11 Application for your child. After answering all questions, click on the CREATE
FORM button at the bottom of the page. Print two copies of the completed DS-11 Application on one side of
the paper. Do not sign the DS-11 until you are instructed to do so by an acceptance agent (see STEP II).

2. Proof of U.S. Citizenship

1). Original or certified U.S. birth certificate OR
2). Your child's expired U.S. passport OR
3). Original naturalization certificate

3.  Passport Photos

Two identical 2 x 2 passport photos (color photos with a plain white background) taken within 6 months.

4.  Proof of Departure

Two copies of your travel itinerary or flight confirmation for a foreign destination issued by a travel agent or an
airline. If you need your passport within 6 business days or less, your departure date must be within 2
weeks, or 3 weeks when a visa is needed.

5.  Letter of Authorization

Click here to download and complete Letter of Authorization, which gives Easy China Visa the authority to
submit your child's application and pick up the new passport. Prepare two copies of Letter of Authorization
with your original signature.

6.  Fees, Processing Time, and Payment

1).  U.S. government fee is $140. Make your check or money order payable to U.S. DEPARTMENT OF STATE.
Write your child's full name and date of birth on the check or money order.

2).  Execution fee is $25. This fee is paid to an acceptance agent you are going to use (see STEP II).

3).  Easy China Visa service fee. Make your check or money order payable to Easy China Visa. To pay by credit
card, complete Credit Card Payment Authorization Form and send it with your application.

4). Return shipping fee. Add $25 to your payment made to Easy China Visa for FedEx Overnight return if you
prefer not to include a prepaid envelope for return shipping. Service fee and return shipping fee can be paid
together by one check or money order.

STEP II.  GET APPLICATION DOCUMENTS AUTHORIZED

Both parents or legal guardian must accompany the applicant to appear in person before an acceptance
agent (Acceptance agents are at designated Post Offices and court houses in your local area) with the
documents as listed in STEP I and a proof of identification for the parents, i.e., driver's license. If only one
parent can appear, a notarized Statement of Consent Form, DS-3053, from the other parent must be
submitted to the acceptance agent. Tell the acceptance agent that you are using an expediting service to
"hand carry" your child's passport application to a passport agency for an expedite processing.

The acceptance agent will witness the applicant and one parent to sign the DS-11 application and seal, in
an envelope, the following documents:

1). One copy of the completed and signed DS-11 application;
2). Proof of citizenship;
3). Two passport photos;
4). One copy of proof of departure;
5). One copy of Letter of Authorization;
6). The $140 check or money order payable to U.S. Dept. of State.

The following items should not be placed in the envelope sealed by the acceptance agent (they need to be
sent to Easy China Visa outside the sealed envelope):

1). The second copy of the completed and signed DS-11 application
2). The second copy of proof of departure - travel itinerary
3). The second copy of Letter of Authorization
4). Your payments to Easy China Visa
5). Printed Easy China Visa Online Order Form

You need pay the acceptance agent $25 execution fee. The sealed envelope will be returned to you.

STEP III.  SHIP COMPLETE APPLICATION DOCUMENTS TO Easy China Visa

U.S. Passport Agency must receive the dated application within 5 business days. Therefore, we suggest you
to send the following documents to Easy China Visa on the same day when you have all the documents sealed
by an acceptance agent using USPS Express or FedEx Overnight:

1). The sealed envelope from the Acceptance Agent
2). The second copy of DS-11 application
3). The second copy of travel itinerary
4). The second copy of Letter of Authorization
5). Payment to Easy China Visa
6). Printed Easy China Visa Online Order Form.

For 24 Hours Processing Only:

1.  All applications must be received by our office before 8:00 AM.  We
recommend that you use FedEx First Overnight to send us your
applications.
2.  Call us to make a reservation for your guaranteed spot.
